Michael David Mathis, 41, a native of Texas and a resident of Foley, died Friday, June 24, 2011 in Bay Minette.
Survivors include his father, Louis (Teresa) Mathis of Tyler, Texas; mother, Jeanne Harper of Atlanta; three children, Jacob, Hannah and Gracie; two brothers, Jack Mathis of Los Angeles and Tony Harper of Atlanta; sister, Lana Mathis of Chicago; grandfather, Bill Green of Camino, Calif.; grandmother, Dorothy Weehunt; other family members and friends.
A private memorial service was held at Faith Tabernacle Church.
Arrangements are by Wolfe-Bayview Funeral Homes & Crematory Inc., Fairhope.
